How do you qualify?
At this time, SIGHTLINE is only available to Medicare-eligible patients and those who wish to pay privately.
Managed care insurance plans do not offer coverage to the best of our knowledge.
In order to qualify for Medicare reimbursement, the patient must:
- Be eligible for Medicare home health. Please visit www.Medicare.gov for more information.
- Have visual acuity of 20/70 or worse (higher) in the better seeing eye with magnification.
- Or suffer from a field loss of 20% or more (field cut).
SIGHTLINE is exclusively and conveniently delivered in your home, in eight weeks.

Your SIGHTLINE representative can help you check your Medicare eligibility and determine if SIGHTLINE can be of benefit.
You will need to provide your Medicare number and the name & number of your eye care doctor.
Medicare does require a script from your primary or eye care physician authorizing the delivery of SIGHTLINE.
We will obtain this script directly from your physician once it is clear that you are eligible.
